Peninsula Special Interest Lions Club Vision 2024 Volunteer Registration

Thank you for expressing interest in volunteering with the Global Initiative for Health and Wellness in San Francisco on Saturday, March 16, 2024.

The clinic will be held at the Presbyterian Church in Chinatown, at 925 Stockton Street, in San Francisco, Right next door to T line Rose Pak underground MUNI station and within 2 blocks of the 30, 45 and 8 MUNI bus. Parking is extremely limited and for volunteers only, public transportation is suggested..

You will receive an email with a copy of your responses to this form. By providing us with an accurate count of volunteers who will be helping us each day, you ensure that we are able to provide the needed services in a timely manner.

The church has a strict mandatory mask policy. All volunteers must submit proof of covid vaccination to [email protected] or via text to 415 377 6467 prior to volunteer shift. Please cancel if you are unwell on clinic day.

The clinic will be open to the public from 9:00am to 3:00pm. Volunteer hours will be from 8:00am to 4:00pm
